ABSTRACT

Abstract Pseudoaneurysm of the ascending aorta (PAA) is a hazardous and potentially fatal cardiovascular disease. This condition is caused by the rupture of at least one layer of the vessel and contained by the remaining vascular layers or the surrounding mediastinal structures. We presented the surgical treatment of a patient with sepsis and large PAA and brachiocephalic trunk, which was compressing the brachiocephalic trunk leading to syncope.

Resumen Objetivo: El objetivo del trabajo es presentar una opción terapéutica adecuada para los pseudoaneurismas de la femoral profunda secundarios a trauma penetrante, así como realizar una revisión de la literatura sobre el manejo en estas patologías. Caso clínico: Paciente masculino de 21 años quien 5 meses previos a su valoración sufre una herida por arma punzocortante en el muslo izquierdo, desarrollando aumento de volumen el sitio de la lesión, dolor y limitación al movimiento. Se diagnostica un pseudoaneurisma de la arteria femoral profunda de 2,3 cm x 2,1 cm x 2,7 cm y un hematoma adyacente de 13,5 cm x 12,6 cm x 23 cm. Se realiza exclusión del pseudoaneurisma mediante cirugía endovascular con coils, posteriormente se evacúa el hematoma adyacente. Resultados: El paciente egresa al tercer día posoperatorio con mejoría de la sintomatología, antibioticoterapia y analgesia. Discusión y Conclusión: En el caso presentado la exclusión del pseudoaneurisma mediante coils facilitó el control de éste y la evacuación del hematoma adyacente, disminuyendo el riesgo de sangrado. Por lo que consideramos adecuada esta conducta terapéutica en pseudoaneurismas de la femoral profunda.


Aim: The aim of this paper is to present a case of a deep femoral artery pseudoaneurysm secondary to a penetrating trauma in the left thigh, its management and a literature review. Clinical Case: 21-year-old male referred to the emergency department of our institution 5 months after he was injured with a knife on his left thigh, with severe local swelling, local pain, and difficulty to the mobilization of the left leg. A 2.3 cm x 2.1 cm x 2.7 cm deep femoral artery pseudoaneurysm was diagnosed with a 13.5 cm x 12.6 cm x 23 cm adjacent hematoma. Endovascular exclusion was made with coils and evacuation of the hematoma with open surgery. Results: The patient was discharged on the third day postop without pain and walking with antibiotics and follow-up to a month did not reveal any complications. Discusion and Conclusion: Exclusion with coils is an adequate management in deep femoral pseudoaneurysms that facilitates the evacuation of the hematoma lowering the risk of bleeding.

ABSTRACT

Resumo Os aneurismas de artérias viscerais (AAVs) e pseudoaneurismas de artérias viscerais (PAAVs) são condições raras e potencialmente letais quando rotos. Em geral, são encontrados incidentalmente na tomografia computadorizada de pacientes assintomáticos. Embora a cirurgia aberta convencional seja considerada atualmente o tratamento padrão ouro, a abordagem endovascular vem ganhando relevo por ser considerada um procedimento minimamente invasivo e com riscos cirúrgicos menores. Destaca-se, nessa abordagem, o uso de embolização por molas (coils) em AAVs e PAAVs saculares e implante de stent modulador de fluxo como alternativa de tratamento para aneurismas fusiformes. Apresentamos o caso de uma paciente de 51 anos com queixa de dor abdominal aguda, taquicardia e hipotensão, com evidência de sangramento abdominal em angiotomografia e diagnóstico de pseudoaneurisma de artéria ileocólica (AIC) roto. Ela foi submetida a tratamento endovascular precoce de embolização da AIC com sucesso, e houve melhora do quadro clínico.


Abstract Visceral artery aneurysms (VAAs) and visceral artery pseudoaneurysms (VAPAs) are rare conditions and are potentially lethal when they rupture. They are usually found as incidental findings on computed tomography (CT) scans of asymptomatic patients. Although conventional open surgery is currently considered the gold standard treatment, the endovascular approach has gained prominence as a minimally invasive procedure with lower surgical risk. In this approach, use of coil embolization in saccular VAAs and VAPAs and implantation of flow-modulating stents constitute alternative treatments for fusiform aneurysms. We present the case of a 51-year-old female patient complaining of acute abdominal pain, tachycardia, and hypotension, with evidence of abdominal bleeding on CT angiography, who was diagnosed with a ruptured ileocolic artery (ICA) pseudoaneurysm. She underwent early endovascular treatment for ICA embolization, which was successful and achieved clinical improvement.

ABSTRACT

El pseudoaneurisma se define como un hematoma pulsátil repermeabilizado, encapsulado y en comunicación con la luz de un vaso dañado. Se origina cuando hay una disrupción de la pared arterial. La hemoptisis es un signo/síntoma de presentación muy rara de aneurisma aórtico torácico y de pseudoaneurisma aórtico torácico. Hay poca información sobre la hemoptisis asociada con la ruptura del aneurisma aórtico cuyo mecanismo no se explica por la presencia de una fístula aortopulmonar. Entre las hipótesis para explicar este fenómeno, se encuentra la capacidad de las arterias bronquiales de volverse hiperplásicas y tortuosas en presencia de una lesión que modifica la arquitectura pulmonar, siendo más susceptibles a la ruptura. También hay descripciones de lesiones directas del parénquima pulmonar por aneurisma roto. El presente caso nos ilustra que debemos considerar a la hemoptisis como signo de alarma en el diagnóstico diferencial de los aneurismas y pseudoaneurismas aórticos entre otras causas que puede ser fatal en breve tiempo por una hemorragia masiva.


Pseudoaneurysm is defined as a reperfused pulsatile hematoma, encapsulated and communicated with the damaged vessel's lumen. It originates when there is a disruption of the arterial wall. Hemoptysis is a very rare sign/symptom of a thoracic aortic aneurysm or pseudoaneurysm. There is little information on hemoptysis associated with aortic aneurysm rupture, whose mechanisms are not explained by the presence of an aortopulmonary fistula. Among the hypotheses to explain this phenomenon, is the ability of the bronchial arteries to become hyperplasic and tortuous in the presence of a lesion that modifies the pulmonary architecture, being more susceptible to rupture. There are also descriptions of direct lung parenchymal injury from ruptured aneurysm. The present case illustrates that we must consider the hemoptysis as a warning sign in differential diagnosis of aortic aneurysms and pseudo aneurysms, among other causes, that it can be fatal in a short time due to massive hemorrhage.

ABSTRACT

El pseudoaneurisma arterial es la dilatación de un vaso, producto de la lesión de la pared. Es generado, principalmente, por traumatismos y, en menor medida, por patologías inflamatorias del endotelio. Se presenta como un hematoma pulsátil y doloroso. Su diagnóstico suele realizarse debido a que, ante una ecografía Doppler, se observa una imagen hipoecoica adyacente a un vaso con flujo en su interior. Su baja prevalencia, asociada a su presentación clínica variable, puede generar confusión con infecciones de piel y partes blandas o trombosis. El manejo puede ser desde la compresión extrínseca hasta la cirugía abierta, y no existen algoritmos terapéuticos en la actualidad. Se describeel caso de un paciente de 13 años con un pseudoaneurisma en una rama muscular de la arteria femoral superficial, secundario a un traumatismo cortante en el que se realizó exitosamente el abordaje endovascular con colocación de microcoils para la exclusión del saco pseudoaneurismático.


Pseudoaneurysm or 'false aneurysm' is defined as an abnormal arterial dilatation produced by an injury to its wall that does not affect the three parietal layers like in 'true' aneurysms. In general, false aneurysms are related to traumatisms and, less frequently, to inflammatory disease of vascular endothelium. Clinically, it shows a pulsatile, painful hematoma in the affected region. The initial diagnosis is usually achieved by Doppler ultrasound showing a hypoechoic image in relation to a blood vessel or its wall. Due to the low prevalence of false aneurysm, it is commonly confused with skin and soft tissue's infections or with thrombosis. There are different options of treatment, from extrinsic compression to open surgery. We describe the case of a 13-year-old patient with traumatic false aneurysm of a muscular branch of femoral artery, successfully managed with endovascular exclusion of the lesion with microcoil

ABSTRACT: We report the case of a 76-year-old male patient with a history of ST-segment elevation myocardial infarction (STEMI) for 3 years. He was admitted to the Emergency Room with a new chest pain episode that began 40 days before and was diagnosed with left ventricular (LV) pseudoaneurysm through 3D transthoracic echocardiography and cardiac magnetic resonance imaging scans. The patient underwent angiography of the coronary arteries, identifying lesions with a multiarterial pattern. Surgical treatment and LV aneurysmectomy were performed with good clinical evolution.


RESUMO: Relatamos um caso de paciente do sexo masculino, de 76 anos, com antecedente de infarto agudo do miocárdio com supradesnivelamento do segmento ST (IAMCSST) há 3 anos. Foi admitido na unidade de pronto-socorro com nova dor torácica de início há 40 dias, sendo diagnosticado com pseudoaneurisma do ventrículo esquerdo por meio de exames de imagem (ecocardiograma transtorácico tridimensional e ressonância magnética cardíaca), tendo sido submetido a estudo angiográfico das coronárias, com identificação de lesões com padrão multiarterial. Foi realizado tratamento cirúrgico, aneurismectomia do ventrículo esquerdo, com boa evolução clínica.

ABSTRACT

SUMMARY Hemorrhagic pseudocysts with pseudoaneurysms are a rare and fatal complication of chronic pancreatitis due to the erosion of pancreatic to peripancreatic arteries. The timing of the rupture cannot be accurately predicted, but prompt diagnosis and management are essential to prevent further bleeding. We describe the case of a 68-year-old man who presented acute epigastric pain and anemia and had a history of chronic pancreatitis with a pseudocyst. A biliary and pancreas MRI showed an enlarged size of a known pancreatic pseudocyst with internal high signal intensity material. Color-Doppler ultrasonography showed pulsating signals in the pseudocyst, and our final diagnosis was a pseudoaneurysm in the pancreatic hemorrhagic pseudocyst. The pseudoaneurysm was successfully treated with coil embolization of the feeding artery. We report this case of a rare complication of chronic pancreatitis to show that color-Doppler ultrasound is a non-invasive and effective diagnostic tool for pseudoaneurysm, which enables early detection and prompt treatment without the need for invasive diagnostic modalities.

ABSTRACT

RESUMEN El pseudoaneurisma de arteria uterina es una causa de hemorragia puerperal poco frecuente y de gran gravedad cuya principal manifestación es el sangrado puerperal tardío. Esta complicación suele ocurrir de manera tardía tras una cirugía pélvica, aunque también está descrito tras el parto. La prueba de elección para el diagnóstico de esta complicación es la angiografía que permite además su tratamiento en el mismo acto, asociando una menor morbilidad y preservando la fertilidad de la paciente. Presentamos el caso de un pseudoaneurisma de arteria uterina tras cesárea cuya manifestación fue un hemoperitoneo masivo a los 17 días de la cesárea y que se resolvió mediante angiografía con embolización supraselectiva del mismo.


ABSTRACT Uterine artery pseudoaneurysm is a serious complication but an uncommon cause of postpartum haemorrhage which can mainly manifest as a severe bleeding in the late puerperium. This complication usually occurs after pelvic surgery, although it is also described after vaginal delivery. Angiography is the best diagnosis method which joins not only the diagnosis but also the treatment at the same moment, reducing the morbidity and preserving fertility. We report a case of a uterine artery pseudoaneurysm after cesarean delivery whose clinical manifestation was a massive haemoperitoneum after 17 days of the cesarean. It was treated by angiographic selective embolization of uterine artery pseudoaneurysm.

SUMMARY Pseudoaneurysms are rare, but femoral artery false aneurysms have increased in recent decades. They are related to endovascular procedures performed on patients with increased risk for this complication. Pseudoaneurysms generally present with only one neck. This paper describes a femoral artery pseudoaneurysm with two necks that occurred after an endovascular procedure and was successfully treated by duplex-guided fibrin sealant. Pseudoaneurysms are rare, but femoral artery pseudoaneurysms have increased with a discrepant incidence reported from 0.5% to almost 4%, mainly related to the increase of endovascular procedures in recent decades. The double-necked pseudoaneurysm identification was of utmost importance to guide the clinical decision-making and allowed good outcomes for the patient.


RESUMO Os pseudoaneurismas são raros, mas os aneurismas falsos da artéria femoral aumentaram nas últimas décadas. Eles estão relacionados aos procedimentos endovasculares realizados em pacientes com risco aumentado para esta complicação. Os pseudoaneurismas geralmente apresentam apenas um colo. Este artigo descreve um pseudoaneurisma da artéria femoral com dois colos que ocorreu após um procedimento endovascular e foi tratado com sucesso por selante de fibrina guiado por duplex. Os pseudoaneurismas são raros, mas os pseudoaneurismas da artéria femoral aumentaram com uma incidência discrepante relatada de 0,5% a 4%, principalmente relacionada ao aumento dos procedimentos endovasculares nas últimas décadas. A identificação do pseudoaneurisma de colo duplo foi de extrema importância para orientar a tomada de decisão clínica e permitiu bons resultados para o paciente.

Resumen Objetivo: Evaluar la efectividad obtenida con la compresión ecoguiada como primera elección para lograr la trombosis del pseudoaneurisma iatrogénico post estudio o intervencionismo. Material y Método: Estudio retrospectivo observacional de 9 pacientes en quienes se realizó la compresión ecoguiada como primera alternativa de manejo frente a pseudoaneurismas iatrogénicos secundarios a procedimientos diagnósticos o terapéuticos ocurridos entre agosto de 2012 y diciembre de 2015 en el Hospital Regional de Talca. Resultados: De 4.070 procedimientos se presentó la complicación en 9 pacientes, un 0,22%; 7 posterior a procedimientos terapéuticos (0,4%), y 2 en el grupo de diagnósticos (0,08%). La muestra se compone por 6 hombres y 3 mujeres de edad promedio 70 años. El vaso comprometido fue principalmente la arteria femoral con un 77%. Los pseudoaneurismas ocurrieron mayoritariamente posteriores a procedimientos terapéuticos (77%); y el éxito de la compresión ecoguiada se obtuvo en el 67%. No hubo complicaciones asociadas al tratamiento. Conclusiones: La compresión ecoguiada tiene un porcentaje de éxito menor en esta pequeña serie que los obtenidos por otros autores, pero al no presentar complicaciones y por su bajo costo creemos debe mantenerse como primera alternativa.


Objective: To evaluate the effectiveness obtained with echo guided compression as the first choice to achieve thrombosis of iatrogenic pseudoaneurysm after study or interventionism. Material and Method: Retrospective observational study of 9 patients who underwent echo-guided compression as the first management alternative to iatrogenic pseudoaneurysms following diagnostic or therapeutic procedures that occurred between August 2012 and December 2015 at the Regional Hospital of Talca. Results: Of 4,070 procedures the complication was presented in 9 patients, 0.22%; 7 after therapeutic procedures (0.4%), and 2 in the diagnostic group (0.08%). The sample consists of 6 men and 3 women of average age 70 years. The involved vessel was mainly the femoral artery with 77%. Pseudoaneurysms occurred mostly after therapeutic procedures (77%); and the success of the echogenic compression was obtained in 67%. There were no complications associated with treatment. Conclusions : Image guided compression has a lower success rate in this small series than those obtained by other authors, but since there are no complications and because of its low cost, we believe that it should be kept as the first alternative.

ABSTRACT

Los pseudoaneurismas son una causa infrecuente de masa palpable en cabeza y cuello, habitualmente secundarios a procedimientos intervencionales; trauma e infeccioso son casos aislados. Caso Clínico. Se presenta el caso de un niño de 14 años que luego de haber sufrido un trauma contuso cortante en la región preauricular izquierda intervenido quirúrgicamente, desarrolla al mes un aumento de volumen pulsátil tras un trauma menor en la zona. En la ecografía se aprecia el signo del yin yang, indicador de pseudoaneurisma. Se realiza en pabellón el vaciamiento del pseudoaneurisma y posterior sutura vascular sin incidentes. Discusión. Pese a la baja frecuencia de pseudoaneurisma como causa de masa en cabeza y cuello, se debe considerar como diagnóstico diferencial en el contexto de masas pulsátiles post traumáticas, de horas a días de evolución. El estudio de elección es la ecografía doppler y el Gold Standard del manejo es quirúrgico con sutura vascular.


Pseudoaneurysms are an infrequent cause of palpable mass in the head and neck; usually secondary to invasive procedures; trauma and infectious causes are rare. Clinical Case. We present the case of a 14-year-old boy who, after suffering a blunt contusive trauma in the left preauricular region surgically treated, develops a month later a pulsatile volume increase after a minor trauma in the area. Ultrasound shows the yin yang sign, indicator of pseudoaneurysm. The emptying of the pseudoaneurysm and subsequent vascular suture was performed without incident. Discussion. Despite the low frequency of pseudoaneurysm as a cause of mass in the head and neck, it should be considered as a differential diagnosis in the context of post-traumatic pulsatile masses, from hours to days of evolution. The study of choice is Doppler ultrasound and the Gold Standard treatment is surgery with vascular suture.

ABSTRACT

Abstract Pseudoaneurysm secondary to chronic pancreatitis is a rare complication, but one with a high mortality rate. It is etiologically associated with chronic pancreatitis, and most diagnoses are made after rupture, which manifests with clinical signs of acute hemorrhage. Computed tomography plays an important role in diagnosis, but digital subtraction angiography remains the gold-standard method for diagnostic confirmation and for treatment planning. This article describes two cases of pseudoaneurysm in patients with chronic alcoholic pancreatitis; one involving the splenic artery and the other the gastroduodenal artery, complicated by thoracic and abdominal bleeding respectively. Both were successfully treated, using minimally invasive endovascular methods to implant coils and stent-grafts.


Resumo O pseudoaneurisma decorrente de pancreatite crônica consiste em complicação rara, porém com alta taxa de mortalidade. Está etiologicamente associado à pancreatite crônica, e seu diagnóstico é feito mais comumente após ruptura, manifestando-se através de sinais clínicos de hemorragia aguda. A tomografia computadorizada desempenha papel importante no diagnóstico; contudo, a angiografia por subtração digital mantém-se como método padrão-ouro para confirmação diagnóstica e direcionamento do tratamento. O presente artigo relata dois casos de pseudoaneurisma em pacientes com pancreatite crônica alcoólica, sendo um da artéria esplênica e outro da artéria gastroduodenal, complicados com sangramento torácico e abdominal respectivamente. Ambos foram submetidos a tratamento endovascular minimamente invasivo com sucesso, através de implante de molas e de stent-grafts.

O pseudoaneurisma da artéria temporal superficial é um evento raro. Ocorre principalmente em homens jovens em decorrência de traumatismo craniano fechado. A maioria dos casos é assintomática, podendo eventualmente haver associação com sintomas vagos. A rotura do pseudoaneurisma e o desenvolvimento de déficits neurológicos são complicações esporádicas. Os autores relatam três casos de pseudoaneurisma de artéria temporal superficial pós-traumatismo craniano fechado. Os pacientes foram manejados com sucesso por ligadura e ressecção dos pseudoaneurismas


Pseudoaneurysms of the superficial temporal artery are rare events. They primarily occur in young men as a consequence of blunt head traumas. The majority of cases are asymptomatic, although there are sometimes vague symptoms. Ruptured pseudoaneurysms and development of neurological deficits are occasional complications. The authors describe three cases of pseudoaneurysm of the superficial temporal artery after blunt head trauma. Patients were successfully managed with ligature and resection of the pseudoaneurysms
